Healthy Start Services
 
The Community Partnership helps individuals
access necessary and important community services. We offer the following:

 Care Coordination/Case Management - coordinating services via case managers to ensure women and their babies have access to the proper medical and social services they need (prenatal care, infant health care, etc.).
 Referrals - linking individuals to the appropriate health and/or social service agency per their request to address specific needs such as financial aid, housing, and food.
 Parenting Education - offering education to parents through monthly classes, home visits, and special events/seminars.
 Family Planning - providing education and information about pregnancy prevention to ensure that women are healthy before beginning a pregnancy.
 Health Education - offering a variety of health education classes through collaboration with local agencies/organizations in our community.
 Outreach - actively reaching out in the community to clients who may be in need of our services, and assisting case managers in providing family support.
 Transportation - identifying transportation needs for scheduled medical appointments and to attend Community Partnership activities.
 Parent Resource Room - offering parenting information, employment preparation assistance and community resource information to families.
 Community Involvement - involves individuals in Community Partnership activities and gives them the opportunity to be an active voice in the community.
